Can I buy a collision report?

If you've been in a collision, you can request a copy of the collision report online or by completing the Request for Collision Report (SCDMV Form FR-50) and mailing it to the address below or taking it to any branch. This report is more detailed than the one you received at the scene of the crash.

If you were in a collision before May 21, 2019, you will not be able to buy the collision report online.

To buy a collision report online, you must login and enter additional information that identifies the collision.

If the collision report has not yet been added to your official driving record, you will be buying a collision report that is stamped with "unofficial."

If the collision report you're buying is on your driving record, it will not have a stamp. When you're completing the transaction below, you will see a message telling you to accept that you understand these terms.

If you buy a collision report stamped with "unofficial," you must pay an additional fee to buy an "official" copy when it's added to your driving record.

Buy a collision report by mail or in person 

When completing the Request for Collision Report (SCDMV Form FR-50), you should include as much of the following as possible:

  • Date of request
  • Your name and mailing address
  • Date of the collision
  • County of the collision
  • Name(s) of driver(s) involved and driver's license number (if applicable)
  • Your signature

Your Request for Collision Report (SCDMV Form FR-50) does not need to be submitted to the Communications Office. You can simply complete the form and mail it to the address on the form.

Once you complete the form, mail two copies of it and a check or money order for $6, made out to the SCDMV, to the address below:
