Can I buy a collision report?
If you've been in a collision, you can request a copy of the collision report online or by completing the Request for Collision Report (SCDMV Form FR-50) and mail it to the address on the form or take it to any local branch office. This report is more detailed than the one you received at the scene of the collision.
Buy a collision report online
If you were in a collision before March 22, 2012, you will not be able to buy the collision report online at this time.
To buy a collision report online, you must enter your personal information authenticating who is requesting a copy of the collision report and enter additional information that identifies the collision.
If the collision report has not yet been added to your official driving record, you will be buying a collision report that is stamped with "unofficial." If the collision report you're buying is already on your driving record, it will not have a stamp at all. When you're completing the transaction online, you will see a message telling you to accept that you understand these terms. You must select the box to accept the terms.
If you buy a collision report stamped with the "unofficial" stamp, you must pay an additional fee to buy an "official" copy when it's added to your driving record, if you still need an "official" copy. SCDMV will not automatically send out the "official" copy once it is added to your driving record.
When completing the Request for Collision Report (SCDMV Form FR-50), you complete this form with as much information as possible. This is a two-page document because the SCDMV wants two copies of your request. If you complete the first page electronically before printing, the same information will automatically populate on the second page. Your signature will not automatically duplicate on the second page, so please sign both pages after printing.
Your Request for Collision Report (SCDMV Form FR-50) does not need to be submitted to the Communications Office. You can simply complete the form and mail it to the address on the form.
