- The self-certification designation associated with your most recent medical certificate.
- The medical examiner information tied to your most recent medical certificate.
This transaction is intended for commercial driver’s license holders in addition to commercial beginner’s permit holders licensed in South Carolina.
To submit your most recent medical certificate, please visit your local DMV office. Alternatively, you may submit a copy of your medical certificate along with a completed Form DL-405A via:
